Liability Waiver & Assumption of Risk

I understand that participation in classes, training, and events involves inherent risks, including the risk of physical injury. In consideration for being allowed to participate, I assume all such risks and release the organization, its instructors, and staff from any claims arising out of participation, except for gross negligence or willful misconduct. I confirm the participant is in good physical condition to take part. [SAMPLE consent text for testing — replace with your studio's own reviewed wording.]
