Liability waiver Athletics Participation Waiver & Release of Liability
In consideration of being permitted to attend and/or participate in this school
athletics event, I acknowledge and agree to the following:
1. ASSUMPTION OF RISK. Attendance at and participation in athletic events
involves inherent risks of injury, including serious injury. I knowingly and
voluntarily assume all such risks.
2. RELEASE. I release and hold harmless the school, its athletic department,
staff, volunteers, and event organizers from any and all claims arising out of
my (or my child's) attendance or participation, except for gross negligence.
3. MEDICAL AUTHORIZATION. In the event of injury, I authorize event staff to
secure emergency medical treatment as reasonably necessary.
4. CODE OF CONDUCT. I agree to follow all facility rules and the direction of
event staff, and understand I may be removed for unsafe or disruptive behavior.
By signing below I confirm I have read and understood this waiver and agree to
its terms. If the participant is a minor, a parent or guardian must sign.
